MFP

MFP – Money Follows The Person

The MFP waiver assists individuals transitioning from long-term care facilities to home-based care. It offers various support services to help make this transition smooth and sustainable.

Eligibility Criteria

You must be transitioning from a long-term care facility, such as a nursing home or hospital, back into the community.

You must be eligible for Medicaid (MassHealth) and meet clinical criteria for home-based care.

TBI

TBI – Traumatic Brain Injury Waiver

The TBI waiver provides community-based services for individuals who have sustained traumatic brain injuries. These services are designed to offer personalized care and support, allowing participants to continue their recovery at home while maintaining independence.

You must be diagnosed with a traumatic brain injury.
